North Central Community Care Hub (CCH) connects people in Chelan, Douglas, Grant, and Okanogan counties to community resources, social service providers and health services that match their unique needs — such as childcare, education, eldercare, food assistance, housing resources, mental health care, primary care, substance use support, tribal health programs, government assistance programs, and more — ensuring everyone in our region has the support they need to thrive.

How does North Central CCH work?

When you complete the referral form, we’ll connect you with a Community-Based Care Coordinator who offers local, in-language, holistic, and culturally responsive support. Your Care Coordinator will work with you one-on-one to link you to the resources and services that fit your needs. They’ll also check in with you regularly to make sure you’re getting the support you need — for as long as you need it.

Thanks to our interpreting services, we can accommodate any language.

Who is eligible for North Central CCH services?

Everyone is eligible, regardless of insurance or legal status.

We serve residents in Douglas, Grant, Okanogan, and Douglas counties — individuals and families, young and old. Our trained Community-Based Care Coordinators provide care coordination that is local, in-language, holistic, and culturally appropriate. The support we provide is free of charge to residents. We may refer you to other organizations for services that meet your needs and if those services come at a cost, your Community Based Care Coordinator will help you plan for and navigate those expenses.
